Detailed Application Instructions

1.    Fill-out the attached “IIS Application
the attached “IIS Budget Detail” 

3.    Create and sign a hardcopy IIS request letter and send to the IIS administrator
       by email or mail. 
       The request letter must include the following information:

o A statement requesting the funds and a short description of the IIS request
o A statement declaring to whom the funds should be paid to (provide contact and 

   address details).
o Wet ink signature by the appropriate representative
o The subject line of the letter should read as follows: “Subject: IIS Request; 
   <Product Name>; <study title >"
o Include full contact, phone, fax, address and email information.
o Attach completed IIS Application and Budget Detail forms.

4.    E-mail the "IIS grant application" and "IIS grant budget detail" to the address
       listed below.
5.    E-mail or mail the signed grant request letter to the address below.
