Requirements

•    • Eisai utilizes an independent medical education agreement for all approved
       grants 

•    • Eisai will not provide funding, or be responsible for funding any grant request
       until: (1) a complete grant application has been properly submitted; (2) the
       application has been approved by the Medical Education Committee; (3) an
       agreement between Eisai and the provider, in a form acceptable to Eisai, has
       been executed.

•    • Grant requestors must complete the Eisai budget template included with the
       application

•    • Grant recipients are required to provide a completed budget reconciliation,
       (utilizing the provided Eisai budget reconciliation form), within 90 days upon
       completion of the activity. Any unused funds must be returned to Eisai
       (via check) along with the reconciliation form
